Yearly visits to the Primary Care Professional involve measuring physical characteristics, such as body mass index (BMI), blood pressure, cholesterol, glucose level, and more. The reason for providing these yearly screenings is that early detection of diseases and assessment of health risks can help the PCP become aware of and more effectively treat health conditions. Empower members that are 21 and younger will be involved in the Early Periodic Screening Diagnosis and Treatment (EPSDT) program as defined by the Medicaid program. If you’re a parent or guardian, ask your child or adolescent’s doctor about the EPSDT process. After seeing your PCP and they have identified some chronic conditions, an Empower nurse may contact you about the Chronic Condition program.
