Chronic Condition Management

The Empower Chronic Condition Management Program is an individualized education and maintenance plans that aim to help Empower members better manage chronic disease and maintain and improve quality of life. The Empower health professional team, may include physicians, nurses, and pharmacists, to educate our members on an ongoing basis about how to better manage their conditions.   The program focuses on diabetes, some heart and respiratory chronic condition.
