Empower has a specialized Intellectual and Developmental Team. You will be assigned a Care Coordinator based on their IDD specialty and the area where you live. The IDD team will assist you with all of your Care Coordination needs. Your CC will also help you with applying for the CES Waiver waiting list. Your CC will help you keep your Medicaid Waiver eligibility. Your CC will help you get specialized waiver services based on your medical needs.
The Community Employment Support (CES) Waiver is a state and federally-funded program for individuals with Developmental Disabilities prior to the age 22. CES Waiver helps members with an IDD diagnosis live in the community instead of an institutional setting. CES Waiver gives individuals and families more choices about how and where they receive services.

Once you are approved for CES waiver services, you may be eligible to receive the following specialized services:
- Respite- Respite services are provided on a short-term basis to members unable to care for themselves due to the absence of or need for relief of nonpaid primary caregivers.
- Supported Employment- Supported Employment services offer ongoing support in order to assist members with obtaining competitive integrated employment in the community.
- Supportive Living- Supportive Living assists the members with acquiring, retaining, or improving skills in a wide variety of areas that directly affect the person’s ability to reside as independently as possible in the community.
- Community Transition Services- Community Transition services are non-recurring set-up expenses for members who are transitioning from an institutional or another provider-operated living arrangement to a living arrangement in a private residence.
- Supplemental Support-Supplemental Support helps improve or enable the continuance of community living. This service is based on demonstrated needs that as problems arise could cause disruption to member’s services, placement, or place him or her at risk of institutionalization.
- Consultation Services- Consultation services are direct services that assist members, guardians, and service providers in carrying out member’s person-centered service plan.
- Specialized Medical Supplies- These supplies are necessary for life support or to address physical conditions along with ancillary supplies and equipment necessary for the proper functioning of such items.
- Crisis Intervention Services-Services delivered in the member’s home or other local community site by a professional. These services include screening, assessing, and developing crisis prevention and intervention plans along with assisting staff in the implementation, monitoring, reassessment, and modification of the plans.
- Environmental Modifications- Environmental modifications are made to or at the member’s home in order to ensure the health and safety of the member or that will enable the member to function with greater independence.
- Adaptive Equipment- An item or piece of equipment that is used to increase, maintain, or improve functional capabilities of members to perform daily life tasks that would not be possible otherwise.
